2–3 Minute Talk Script
Confined space safety does not end when the work task is complete. Entry termination and closeout are important because workers, tools, equipment, and energy controls must be accounted for before the space is returned to normal service.
Before closing the entry, all entrants should be accounted for. No permit should be closed until everyone who entered the space is confirmed out.
Tools, hoses, cords, monitors, ventilation ducts, lighting, ladders, and temporary equipment should be removed or accounted for before the space is closed.
Energy controls, valves, covers, hatches, and process systems should not be restored until workers are clear and the responsible person authorizes the change.
Barricades, signs, traffic control, and access controls should remain in place until the entry is complete and the area is safe for normal use.
The permit should be closed according to procedure. Any unusual readings, hazards, problems, equipment failures, or near misses should be documented.
Communication is important during closeout. Entrants, attendants, supervisors, operations, maintenance, and affected workers should know when the entry is complete and what systems are being restored.
Safe confined space closeout depends on accounting for people, removing equipment, documenting issues, and restoring systems only after the space and workers are safe.
